There are only three documents we need to complete your appeal.  You can click on the document name to be taken to a fillable PDF version.  You can simply print, sign and return the documents to us by e-mail, fax or mailing them to us. The three documents are:

  • Representation Agreement

    • This is our simple one page agreement that allows us to represent you in your tax appeal and explains how we are paid for our services (only if we save you money).

  • Attorney Authorization​ - Cook County Board of Review

    • This form is necessary for all appeals to be filed​.  This form allows us to represent you in front of the Cook County Board of Review.

  • Attorney/Representative Authorization - Cook County Assessor

    • This form is needed for some appeals, but not others, depending on the date your appeal is filled.  This form allows us to represent you in front of the Cook County Assessor.  NOTE: If you are unsure if you need to fill out this form, we ask that you complete it and send it in to be safe.  You can always contact us and we will be happy to let you know if we need this form in your case or not.
